Inverted tummy tuck complementary to hernioplasty. Case report

Abstract

Background: Abdominoplasty in patients with abdominal hernia is one of the interventions that most frecuently unites plastic surgeons and general surgeons. The reverse tummy tuck is not common and is performed in specific cases of abdominal defects.

 Objective: To present a clinical case of a patient with hypogastric hernia who underwent hernioplasty complemented with an inverted abdominoplasty and to review the updated bibliography.

 Presentation of the case: a 64-year-old female patient with a history of severe obesity, high blood pressure and scarring in the abdomen and lower limbs, from a car accident approximately 24 years ego. She was consulted with plastic surgery by the general surgery Service because surgery was planned for an abdominal hernia located in mesogastric region that required placement of polypropylene mesh and the skin where the defect was located did not have the quality to guarantee closure. She is operated on, performing a hernioplasty with placement of the prosthesis with opening and closing of the skin defect with and inverter abdominoplasty technique. The patient suffered from large seroma as a postoperative complication that was treated and resolved without further difficulty.

 Conclusion: The inverter abdominoplasty is an infrequent reconstruction option considered in cased of hernioplasty of the upper hemiabdomen where optimal skin coverage is required.
