Lipoma, more than an Unsightly Tumor; a Reconstructive Option. Presentation of a Case Enriched with Nanofat

Introduction: Lipoma is the most common benign soft tissue tumor and, although it is generally treated for aesthetic reasons, in some cases it may represent a reconstructive option. Breast reconstruction in patients with acquired amastia is a surgical challenge, and the use of nanofat-assisted autologous flaps can improve functional and aesthetic results.
Objective: To expose the use of lipoma as autologous filling material, not as a graft but in the form of a nanofat-assisted flap.
Case presentation: A 22-year-old female patient with a history of hyperandrogenism, hirsutism, amenorrhea, and unilateral right amastia secondary to surgical removal of the breast bud in childhood. An adjacent thoracoabdominal lipoma was identified that allowed its use as a vascularized flap for breast reconstruction. Under general anesthesia, an incision was made in the previous breast scar, the lipoma was dissected preserving its vascular base and relocated in the breast region to give volume and form the lower pole of the neo-breast. The procedure was complemented with the infiltration of nanofat obtained by emulsification of adipose tissue, which favored tissue regeneration and flap integration.
Conclusions: The use of lipoma as an autologous flap assisted with nanofat proved to be a viable and safe reconstructive option. The combination of these techniques allowed reconstruction with native tissue, improving adaptation and healing, without the need for alloplastic materials.
